Cut bread into 1-inch thick slices, and arrange them on a baking sheet.
Mix remaining ingredients in a small bowl.
Top each piece of bread with the cheese mixture.
Bake until butter is melted and bread toasted, 15 to 20 minutes. If cheese starts to brown before butter is melted, cover sheet with aluminum foil after about 10 minutes. Serve warm.
